The game's gameplay is similar to that of the previous installment, with one key difference: 47 can now disguise himself by donning a variety of disguises and gear. In the first mission, the player has to infiltrate a mine and rescue a trapped miner. The second mission involves the assassination of a German scientist named Dr. Doppelmeyer, who has been kidnapped by the terrorist group and is being held in a meat locker. The third mission involves the kidnapping of Dr. Ort-Meyer's niece, Mariana, who has been kidnapped by the same terrorist group. The fourth mission involves the assassination of a German politician, and the last mission involves the rescue of Emilio Vittorio. After each mission, 47 can choose to share details of his actions with a priest or escape out of a mission's location with his bodyguard Blume. The mission can be replayed from the beginning without the knowledge of the previous game's plot line.
The plot unfolds when a German company, Ort-Meyer, which is building a small fusion reactor, is abducted by nuclear terrorists. Ort-Meyer is unable to assist the authorities because their designer, Dr. Ort-Meyer, is missing and the only surviving witness, Emilio Vittorio, is nowhere to be found. 47, the game's protagonist, is hired to locate Ort-Meyer and to take out the terrorists. On his quest, he encounters a number of situations similar to those of Codename 47 and makes different decisions as to how to proceed. 47 is armed with a Pistol, Body Armor, Grenade Launcher, Pistol, a set of Leather Gloves, a pair of Leather Pants and a set of Metal Shoes with a Messer Knife. 47's weapon arsenal, as well as his ability to disguise himself with a variety of disguises, are customizable to his liking. 47 has two weapons at his disposal: a silenced M67A1 Compact Assault Rifle, and the M67A1 Compact Sniper Rifle.
The game takes place in Italy. The game is set in an alternate history where a terrorist attack has occurred in Rome. A terrorist organization called the Ghost Protocol has taken over Rome. This terrorist organization is formed by the same person who is responsible for the assassination of Dr. Ort-Meyer. This person is known by the public as the 47. The 47 is a black man and he has a specific code name given to him by the military. They call him as the Assassin because of his ability to kill anybody and get away without being seen.
